> STEPS BEFORE SUBMITTING YOUR DONATION REQUEST - 

* Review the Event Date Range and corresponding Submission Deadlines before you fill out your donation request - view event date ranges & submission deadlines below 

* If you the donation deadline has past, please do NOT fill our a request, as we will not be able to process your submission. We kindly request that you check back with us next year 

* All Donation Request must submit a Donation Request Letter and In-Kind Donation Form in order to be considered | IRS Tax Exempt Forms are NOT accepted   

Due to the high volume of requests we receive, we must adhere to the above guidelines and our provided submission deadlines, as this ensures a fair and efficient process for all parties involved.

* If you have not heard from us, please do not follow up on your donation submission until the month prior to your event date

* Our Donation Gift Cards are valid at any Lombardi Family Concepts restaurant regardless of location
